Larger doses of naloxone are appropriate in the setting of a drug overdose. If one is reversing narcotic postop then a light touch is in order as the sudden reversal of or complete loss of pain control can precipitate a large jump in blood pressure potentially causing a stroke or heart attack. In the setting of a possible opioid overdose you are treating impending loss of breathing and avoiding cardiovascular collapse. Naloxone has little effect on a person who does not have opioids in their system to reverse. It's a pretty benign drug overall that can be extremely dangerous in a very narrow setting.

To the paramedic's point, titration in the context of an acceptable monitored blood pressure and managed airway is perfectly fine. My understanding is this product is intended to be used by lay people in the field with no monitoring, no equipment and little or no training. In that setting a large dose would be the most likely path to a good outcome.
