…current rules for Medicare and Social security mean quite a large commitment for the future. What will change? Maybe less benefits, changing the commitments?

For Social Security, the “normal” retirement age—i.e. the age at which standard benefits begin—can be gradually increased. This has already been done, and it will undoubtedly be done again.
