From what I understand. Silicon Nitride leaches away the Nitrogen atoms to form multiple molecules including peroxynitrite and Orthosilicic acid then forms a hardened silicon dioxide surface layer. We dont fully understand the role silicon plays in bone health but 70% of your bioload is stored in bone and animals studies deprived silicon have shown osteoporosis, leaving little doubt it performs an essential role in bone health.

The ability for uptake of the silicon dioxide particulate faster than wear occurs probably helps flush contaminate out of the sinovial fluid, decreasing the rate of wear as particulate builds up. Think dumping metal shavings in your cars oil and never changing it and what this does to your main bearings. I think your body is saying "I know this, this is familiar when it encounters silicon dioxide" Unlike Aluminum which builds up in your bone replacing an element(My guess silicon) and weakens it. When your bone encounters metal and plastic particulate it doesn't have a use for it. It's detrimental if absorbed. If it cant be absorbed it's detriment is localized as the body attempts to encapsulate it and the bone tries to run away (osteolysis)and a foreign body response is generated into pain and swelling,rejection and possibly infection without a bacterial cause.

Even HA coatings on titanium are delaying the inevitable. Once it converts the HA coating into bone it starts asking itself "is this titanium a bone? or a foreign body?" and it shows a poor bond rate to titanium once in vivo as the body assimilates the coating and begins to reject the foreign body.

This is all my best guess at deciphering what Ive read on the subject anyway.
