It could have been something as simple as a safety item. I my college days I worked with a garment mfg. The sewers hated the safety items like needle guards as it slowed their production up so they or management would remove them. They also had a cover on the belts. So, what if the stacks needed a cover that was missing or removed? Could have been they were removed or maybe a design flaw. If so, inspection or a simple fix might have cost 2.6 million? Just a thought. It could be part that and a set aside for the deductible on their insurance.
