$RXMD After working with patients in the hospital setting, Westchester General Hospital has identified numerous readmission risks stemming from post-discharge/transitional care. Their case workers routinely work with patients to minimize these difficulties but have been dutifully searching for an effective solution to transitional care management. Through Progressive Care’s wholly owned subsidiary PharmCo, LLC, DischargeRX will provide Westchester with much-needed support to prevent unnecessary and costly hospital readmissions. DischargeRX will offer hospital patients and physicians access to a PharmCo representative, who will offer medication-related services throughout the patient’s transitional process.
