It's been some time since I've visited but I may have what I think is the safe and sane solution to your problem. I'd hire an insured person to change out all the lights on the ceiling with beyond-your-lifetime, long-lasting LEDs. Have him change the code on the fan receiver while he's up there. You don't have to buy anything.

Nineteen feet is a long way up. If you decide to do it yourself I'd suggest a parachute.
