What specifically is the SPA endpoint?

Easiest question there: we don't know what the SPA says at all. We can presume it relates to the primary endpoint (disease-free survival), and note their timeframe: 5 years. That's because a lot of patients live disease free for that long.

And you understand fully well what the median means in this context: time it takes for 50% of patients to have recurrence. It is a standard measure of these endpoints, although landmark analyses (ie, 12-month survival, 24-months survival, etc) are also very important.
