OTC stocks, can and do trade during extended hours. Form T actually denotes any trade done outside of regular trading hours, so, it is in fact an extended hours trade.

However, OTC stocks rarely trade during extended hours because it is at the discretion of the broker dealer as to whether or not they allow investors to do so. Typically, this capability is restricted to institutional investors, accredited investors, or a select few high net worth clients.

The primary reason is because of the extremely thin liquidity, and the average investor does not understand the risk.
