Generic 101 question — and maybe this doesn’t come up in practice. But if the doctor writes a prescription for Copaxone or some other brand name but doesn’t write dispense as written, who decides which generic to dispense? For small items like glaucoma eyedrops it seems to be the pharmacist but maybe it’s the insurance company.
