Instructions for sending the certificate into your brokerage account:
1-Enter on the back of cert in box social security number.
2-Print name,address,city,zip and email address if available.
3-Where it says "appoint" enter Etrade Securities as attorney.
4-Date
5-Where it says "in presence of"
you sign your name(s).
6-Where it says signature guaranteed that can be done by your bank but signatures must be signed in front of them.
7-Then mail cert to your brokerage account.
