Simple, really...buyer places an order to buy and MM can't find them all at one seller. So they'll buy the least expensive ones to fill your order, and then get some at a higher price from another seller. Just this year I placed an order for 1M shares and got them at two prices - the lower price got me 999,999 shares and the higher one got me 1 share. Yes, I can see it in my etrade transactions...One share.
