The main reason why i am not sure that oncs treatment will add anything to anti pd1 in stage iv is that electroporation is done to the (sub) cutaneous lesions making them more permeable to the specific drug used. That is the same with electrochemotherapy. You can not electroporate visceral metastates hence any effects we see in these types of lesions is most likely due to the anti pd1
