Here is a good read about HFACS and how their model fits in with medical issues and how they can reduce costs. This should be a great revenue generator for Quture once they have the final software release ready imo.

•HFACS is an analytical tool (part 1). It helps to identify and record sources of human error across organisations. This is particularly valuable for an organisation like Christus Health, which provides services in over forty locations. HFACS is helping them implement a strategic response to eliminating common errors. The same aim was driving MD Anderson’s use of HFACS for investigating ‘sentinel events’ in surgery (roughly the same as NHS England’s surgical ‘never events’). All healthcare providers are vulnerable to the pressure to focus on quickly fixing whatever went wrong last.

HFACS is a practical, tried-and-tested safety management tool based on human factors. You do not need extensive expertise in human factors to use it. It helps you work systematically to identify, analyse and manage the behaviours that are leading to harm in your organisation. It can also help you to identify and reinforce the behaviours that are promoting safety.

HFACS was first developed for use in military aviation. However, US healthcare providers including the Mayo Clinic, the MD Anderson Cancer Centre, and Christus Health saw its benefits. They helped to adapt it so as to make it suitable for healthcare.
