In 2009 Instacare issued millions and millions of shares to company directors , when the stock was trading at a penny. In November 2009 6 to 12 months after these issuances a massive pump occurred bringing the share price to 32 cents under massive liquidity.
Keith m berman regularly states he does not get compensated in this company. If you read the SEC questionnaire after the 2009 stock pump, you will see he did indeed issue millions of shares to himself and the other directors of the company.

It appears that you recognized debt relief other income of $920,379 related to the convertible promissory note issued in March 2004 and $87,309 related to the promissory note issued in February 2005. The sum of these two amounts (i.e., $1,007,688) appears to be inconsistent with the equivalent amount presented in your statement of operations (i.e., $1,450,867). Please clarify or revise.

15.
