I think that Dr West is being an alarmist for many reasons and one is clearly the progression of the disease.

The results of the anti PD-1 drugs are very nice from a stat position both in 1st or 2nd line. But while the are good they certainly don't cure anything.

So the answer has to be Patient management. We now know the PD drugs will extend life after a patient has been on 6 to 10 months of Chemo we have no clue if Chemo will extend life after 8 to 12 months on a PD drugs.

Most of the patients have dire results but to give any PD drug 1st line we have to know what to do next.

The benefit while still being stat sig is meaningless between chemo and the Pd's in 1st line.
