In the 90's under managed care California was able to see pure premiums actually be reduced. The data is out there and if you are interested in reading up on it the book called Health Care Economics by Feldstein is a great resource. The ACA has helped reduce waste significantly. I am not here to talk about what party did this or that. I only talk about things that have HARD DATA to support it. Longitudinal studies that go back and look through EMRs and insurance billing have been able to support these facts.
