If I had my wisdom teeth pulled I'd want the pain to stop ASAP, not 2 hours from now. Hence the term immediate release. On a more relevant note, why do you think addicts extract the opioids from ER formulations and snort/inject? Hint, it's not because they want to get high in a couple hours.

Acid, shrooms and MDMA are a different story.
