You typically do have to ship by air because of the short-half life. You would probably need at least one centralized facility within Europe and/or US to produce the material and then ship to individual hospitals. 64 hour half-life of the material means that the probably have to ship at least twice what the prescribed dose will be to account for decay during shipment. This stuff is on airplanes every day and even on passenger aircraft as well if it is for medical purposes and the correct distances are maintained between the radioactive material and the passengers.
