But where is the SEC filing that confirms this? Did they forget to file it? And where is the 8K detailing the change in accountant?

Item 4.01 Changes in Registrant's Certifying Accountant

According to the SEC's website

Companies have four business days to file a Form 8-K for the events specified in the items in Sections 1-6 and 9 above.

I am pretty sure 4 is between 1 and 6 so why would they purposely ignore this simple SEC filing requirement?
