For the acute septic or otherwise infected inpatient, the possible increase in SBP isn't an issue.

Either the truly sick, septic, patient is already hypotensive, or you can easily manage the increase with a touch of Hydralazine.

For the outpatient, monitoring pre-Rx SBP and instructing the patient to either return to the office for a recheck vs BP cuff at local pharmacy.

Not a big issue.
