FLA - reading b/t the lines perhaps they plan to make v-pak preferred? It looks like at their last meeting they put every hcv drug on the non-pdl list (including harvoni), but v-pak isn't listed at all. http://www.fdhc.state.fl.us/medicaid/Prescribed_Drug/pharm_thera/pdf/Changes_Summary_Report_Jan_2015.pdf
