Many things bother me about SVFC but this is not one of them. Believe me or not, its cookie cutter pre-populated fill in blank with company name standard stuff. CMS, DOH, life safely all have the same sting to their violations. Gov oversight departments don't show up and shake hands and pat each other on the the A$$.
Know it well.
