If they use PET scan they can easily tell. Basically, cancerous tumors consume glucose at a much higher rate than other body tissue and the radioisotope used is fluorodeoxyglucose (FDG), an analogue of glucose. When a cancer cell us dead it no longer consumes glucose and no longer shows ip on a pet scan. ***all that is needed is a baseline scan for comparison to determine if the tumors are growing or shrinking. This type of study is based on bioactivity rather than visual clues and is therefore more sensitive than other imaging modalities. PET scans are combined with CT scans to provide additional information and different cross sectional views and you often find that combination PET CT scanners in use.
