I chuckle reading about daptomycin as I've never seen it used in the perioperative setting. I don't even know if daptomycin is on our hospital pharmacy formulary. Have you personally seen much use of daptomycin? Clearly, a lot of places are using a heck of a lot of it.

As previously noted, Cefazolin (1st generation cephalosporin) is used as antibiotic prophylaxis for most surgeries. Cefoxitin (2nd generation) is used fairly often, especially for certain General Surgery cases. Some penicillin allergic patients are given clindamycin instead of a cephalosporin. These antibiotics are dirt cheap and effective in the perioperative setting. Slight variations in regimens from hospital to hospital.

I think the best perioperative opportunity with brilacidin is for orthopaedic cases where hardware is inserted. Total joint replacements come to mind as these are absolute nightmares when they get infected. Hardware coatings and/or mixing brilacidin with cement (methyl methacralate) might be considered. Also, I think a good case can be made for using brilacidin with other ortho cases where hardware (plates, screws) is inserted.

I am curious as to the infusion time for the single dose brilacidin and if any adverse effects are noted with the low dose administration.
