This is what I got from optionsxpress:

If you want a certificate issued to you there is a fee of $50.00 per certificate issued.The money must be in your account before the instructions can be sent to our clearing firm. You would simply send us an email stating your account number, share quantity, name of issue, the registration if different from the account and the address of where the shares are to be mailed.
