BK can give you the exact definition but here is the layman's version:

If Cohort 9 is at 250mg and they hit SAE's or DLT's (Dose Limiting Toxicity),... they come back to where the previous dosing level was and call that the MTD (Maximum Tolerated Dose),... therefore 210mg.

They then invite the remaining patients (to make a maximum of 40 in all),... to dose at that level.
