SPORT will have an access port instead of a third arm. So a human will be the "third arm" if needed. I think this will be fine and means less crowding for the mechanical parts. The third arm on the DaVinci is used primarily to retract or hold. It doesn't take much to do that. I think in the long term surgical assistants will play this key role in robotics and third arms won't be needed. It's also cost effective.
