I spoke with a Fidelity trading representative earlier today and confirmed that the ballots, for common shares (wamuq), should be mailed to one of the following addresses depending on the type of service used.

In addition:
1. Completed ballots need to be mailed. No other option available.
2. Ballots must be received by Fidelity no later than 2/3/2012 for voting purposes. Later, if just for the purpose of granting releases.
3. If submitting more than one ballot they all can be sent in one envelope.
4. The last batch of preferred ballots (wampq) were mailed on 1/24/2012. I was told that we should receive them shortly.
5. Trying to get an electronic copy (pdf file) of any of the ballots is nearly impossible.
