Intraductal Approach / Ductoscopy
Ductoscopy, also referred to as mammary endoscopy, involves inserting a microendoscope into the ductal opening of the nipple. This allows a clinician to directly visualize the breast duct and, if desired, to perform ductal lavage. Ductoscopy is an emerging technique that has been utilized in the intraoperative setting in patients with both benign and malignant breast conditions in research and clinical trial collaborations.

A ductoscopy procedure begins with application of a lidocaine cream to the nipple. Lidocaine is also used intraductally once the duct has been cannulated. The breast is then massaged to produce nipple aspirate fluid and to allow for identification of the ductal openings. Any duct that yields fluid can be scoped. The selected duct is dilated with small metal wires, and the ductoscope is passed into the duct and advanced into the breast. A camera and imaging system magnify the video images from within the ducts and display sharp, clear pictures on a computer monitor. At this point an intraductal biopsy can be performed or abnormalities can be marked for a surgical or image-guided core biopsy. The ductoscope also has an outer sheath, which allows ductal lavage to be performed for cytological analysis.

Ductoscopy is used during surgery to guide microductectomy, or excision of abnormal breast ducts. It also can be used in conjunction with transillumination, to guide incision placement, or to perform a biopsy, with the ductoscope left at the site of the lesion to guide the surgical excision.
