I am not too excited with new areas as of now - get 1st approval and go for CE mark and start internal trials.

Co has limited funds and can do so much - so focus on key area and then start addressing peripheral areas as they are fraction of internal application. They need to start the next big trial in US for internal surgery - it will be bigger than all other applications together.
