"The first pass effect" refers to drugs that are swallowed and must pass through the digestive system.

Syndros is administered as an aerosolized liquid rather than a pill to go directly into the bloodstream. Some of the liquid dronabinol will inevitably be "leaked" to the digestive system but even in the case of the far more dangerous fentanyl converted to an aerosol Subsys there seemed to be kittle harm.

The idiocy of treating decades-long FDA-approved dronabinol as still a dangerous narcotic demonstrates the terrible puritanical superstition that Insys still faces.

How exactly are we better than the Puritan colonials of Salem who also preferred witchcraft to science and reason?
