When a surgeon declares a complete resection has been performed (100% tumor removal), they are talking about the visible tumor. There are often to almost always clusters of unseen cells left behind. In some cases, such as is common with proneural tumors, these clusters are often located off site from the original tumor and the tumor recurrence is in a different location from the original tumor. Once these clusters become more fully established DCVax-L can not induce an immune response that can overcome the immuno supressed environment created by the recurring tumor by itself. This is where the checkpoint inhibitors can have a chance to help by allowing the immune response to remain active. Best wishes.
