I understand that it is not approved for general use but I thought it was approved for salvage therapy and that would apply nation wide.

Is this a money thing? What is the cost of a course of treatment?

If money is the problem it seems to me that offering cut rate therapy to untreatable victims is both an ethically good thing to do and (enlightened self interest) short circuits the Phase II clinical trials.
