If patients at high risk for arterial occlusive events were purposely put in the low dose group because it was believed that they would fare better on a lower dose that could explain why the lower dose had more events if risk is the overriding issue and not dose. I am assuming this is a real-world population and not a randomized trial. We need more information to draw any conclusions.
