Nice job. After 6th inject they are continued to be monitored, though receive no additional injections (patients may pursue other therapies). As you know safety assess is at 6 months, but tumor response goes out to 18 months, and PFS and OS go out to 24 months. Every 2 months or so the patients undergo CT scans and general evaluation. They have biopsies only at each injection--week 0, 1, 2, 8, 16 and 32. Six biopsies total of the primary injected tumor. Some may request additional biopsies and also biopsies of distal tumors, but it is not a part of protocol (that we know of).

The only thing that may be off about your chart is that the first 3-5 or so will likely have had all six injects by sometime in Nov.

I'd offer you advice, but I just don't care about your money, unless you give me money to care about your money. I might even be tricking you with the above post...
