DOJ contact number on it. They probably have a form that they would like to see used and can provide the details required. THEN you would have to either take the info from your paper accountant statements or online statements and perhaps provide copies along with the form.



I've never been a shareholder in this nonsense, but this board was littered with them back in the day, many of whom are in EXACTLY the same position that you are.

I DON'T KNOW WHY THEY AREN'T SHARING ANY INFORMATION WITH YOU REGARDING THIS PROCESS.
The letter itself implies that a number of affidavits have already been received.
