There are about 300,000 to 400,000 stock brokers in the US. They all not represented on level 2 because they use market makers to trade through. The put orders through these brokers which are called secondary brokers. These secondary market makers will sell or buy at a different price. For example a shareholder may tell their broker to sell at 10,000 shares at $.05 but the secondary market maker may sell the stock at $.0525 and keep the $.0025 and then print the 10,000 sell again at $.05 or if there are a few orders they will print an average sell order at the end of the day.
