None of the above. Were KMAG to miraculously get a Form 211 approved by FINRA, it would begin trading on the Pinks with a bid and ask. But that would mean no Form 10 had been deemed effective. If it had, the stock would trade on the OTCBB, not the Pinks.
And as noted, sponsoring MMs do not approve Forms 211. They file them for the company. FINRA decides whether they'll be approved or not.
The procedure is to get the Form 10 filed and deemed effective first, and then to find a sponsoring MM>
