In general I think with any medical procedure patients are informed of all potential risks and then sign a risk sheet acknowledging all potential risks. And the possibility of lack of improvement or lack of a "minimally clinical important difference" would be part of that informed risk disclosure process. Most doctors I would think are not going to reimburse treatment costs since the risks are usually disclosed to patients in advance. I am just speculating though since I only know general practice in surgical settings but not what is done specifically with this particular treatment.
