As far as FDA requirements go, another poster mentioned that one of the reasons for possible FDA pickiness with the back pain versions would be because of the application sites being directly over organs, whereas foot and knee obviously aren't. Seems like a valid concern to want verify is safe, and sadly, being the FDA, I would expect them to want to milk as much money out of application fees for those different uses as they can. >.<
