Happy to help with this, instead of choosing the "Amendment After Issuance of Stock" be sure to check the "Certificate Pursuant to NRS 78.209" which will show the Reverse Split 1:1000. Here is the image file for your reference. Darn shame there is a split but it is what it is.

You can see the entity numbers match as well :(
