So the NEJM figure also skipped the first step to humanize the antibody. They all start from a mouse antibody, since this is the predominant model used to produce the monoclonal antibodies to begin with. Also, the humanization step will yield the full antibody rather than the Fab fragment, if I'm not mistaken... I'd have to check on that to be sure. That might be a small error in the NEJM figure.

They're really the same thing, but the NEJM figure cuts a few corners. Understandable since few docs would really care how the antibody was made, as long as it is efficacious.
