Between radar, officers, and other posted visual outlooks this is disturbing. The damage to the destroyer was similar to a tin can. Seven lost lives is inexcusable.
The public may never be told.
Forty years ago, the official report said that a Navy man fell overboard on his watch, and was lost at sea. The parents heard otherwise and pressed their Congressman to verify that a hatch on an outside walkway was badly rusted but just kept getting painted over. Their son stepped on the hatch and fell through.
Obviously, maintenance is more than just a coat of paint.
