Some questions about filing a 13G ...

I need to file a Schedule 13G (beneficial ownership)and downloaded the form from the SEC site but the format is confusing and the instructions for completing it don't seem to match the format at all. So:

1) Is the form required or will a document that contains the required data suffice?

2) Can it be filed (or does it have to be filed) electronically? I've seen some indications that it should be filed electronically but searched quite a while last evening with no result in either answering the question or finding instructions on how to do so.

3) Is there a fee for the filing and if so, how much?
