As stated, the dates/ shares/ conversion rates/ et al were all left blank. The only thing I can think is that the form is submitted electronically to Edgar and TDCP did not save it properly or format it correctly before it was sent. It will be much easier to see the devastaing impact on current shareholders once the blanks become real numbers.
