Yes, .003s and .004s are realistic. I've been a part of some giant merger plays and have seen the trip to penny run a few times. Granted, I've normally been in those with O/S under a billion. I do remember one though that had 1.6 B Outstanding that ran from .0002 to 8 cents. I kept shorting myself throughout that run because I never believed it could do something that incredible.
