I was going to write "why go viral on a study based on assumptions since we don't have R-IT data yet, they're just making educated guesses, so who cares", but I reread the article and found this gem:

LIMITATIONS: The model is only an approximation of reality and does not capture all complexities of a real-world scenario without further inputs from ongoing trials. The model may underestimate the cost-effectiveness of EPA-plus-statin therapy because it allows only a single event per patient.

They essentially followed primary event counting rules for R-IT, a major limitation of cost-effectiveness calcs.
