I'd call this statement:

If they can show decent to good results, quickly, out of patient group as small as 18, everything's possible. If not, nothing.

realism, not optimism.

It seems to me that pessimism would be to think that the drug doesn't work at all and that the outcome will not be either decent nor good, and optimism would be to think that they necessarily will be good or better. Personally I'd set the odds of getting a financeable result at about 50/50. It's a casino, don't forget, and best of luck with your bet.
