Reporting Requirements: ANNUALLY (CALENDAR)
Reports due 30 days after the reporting period.
The initial report is due 1/30/2014.
Subsequent reports are due every 12 calendar month(s).

Compliance Demonstration shall include the following monitoring:
Monitoring Type: RECORD KEEPING/MAINTENANCE PROCEDURES
Monitoring Description:
Used Oil (Waste Oil) is to be co-fed along with the
plastic as a heat transfer media to assist in the melting
of the plastic.

The oil is to be fed as a maximum of 0.1
gallon per pound of plastic fed on a 1-hour average basis.
Records of oil and plastic feed rates to each unit,
including a calculation of the hourly average feed ratio,
will be maintained onsite by the facility for three years.



An annual report verifying compliance will be generated
and maintained on site at the facility for review.
